Your First Dental Visit

During your first visit, we’ll make sure you’ve completed all the new patient forms beforehand. If not, we’ll ask you to fill them out, and then bring you back to meet Dr. Goodman and one of our hygienists when we are ready. During your first visit, we’ll make sure to give you enough time to discuss your dental needs directly. If you need a basic checkup, we’ll perform a brief exam and professional teeth cleaning with time permitting. If further treatment is needed, we’ll recommend a treatment plan, and then get you scheduled for a follow-up through our front desk.
New Dental Patient Forms

New patient forms allow us to learn more about you, your oral health needs, and any other accommodations we should know about ahead of time. This includes pre-existing conditions that can affect your oral health, current medications, and contact information. Please download, print, complete, and bring your forms to our office before your visit. Alternatively, you may arrive 15 minutes ahead of your scheduled appointment time to complete these forms.

Dental Insurance Welcome

We gladly welcome all PPO dental insurance plans and are in-network with Delta Dental Premier. You can expect our team to go over your coverage to ensure you’re getting the most out of your dental benefits. Our front desk will read over your plan and do their best to maximize it to the fullest extent. If you have questions about financing your care, please let us know; our knowledgeable team will be happy to go over your options.
